Macrolide Antibiotics Mechanism: Inhibition of bacterial protein synthesis 50 S ribosome ; Protein synthesis occurs continually in all cells, but certain drugs are reasonably selective against this process in bacteria. These drugs bind to specific sites on bacterial ribosomes, which are integral components of protein synthesis. Ribosomes consist of two major components, termed the 30S and 50S subunits. Inhibitors of bacterial protein synthesis achieve selective toxicity because they do not bind appreciably to mammalian ribosomes, and usually do not inhibit protein synthesis in human cells. Macrolides are usually bacteriostatic agents. Although they are particularly effective against G + organisms, several important G- e.g. Hemophilus influenzae, Legionella pneumophilia ; and atypical bacteria e.g. Chlamydia trachomatis ; usually respond. As a result, the macrolides are considered to have an extended antibacterial spectrum. Erythromycin was the first macrolide "large ring" ; antibiotic to be developed, and it is still widely used. But erythromycin is not acid stable and is not completely absorbed from the GI tract. Erythromycin salts stearate, ethyl succinate, estolate ; have been marketed to improve these features. Clarithromycin * Bjaxin ; is stable in acid and is well-absorbed from the GI tract, so it is often used as an alternative to erythromycin. It also has a longer half-life, which allows it to be given less frequently than erythromycin. Azithromycin * Zithromax ; is distributed into certain tissues, and can be given once per day because it is very slowly eliminated from the body. Both clarithromycin and azithromycin have antibacterial spectra that are quite similar to that of erythromycin. Telithromycin Ketek ; is the most recently marketed macrolide-like drug. It has additional binding sites on bacterial ribosomes, and is sometime effective against bacteria that do not respond to the other macrolide antibiotics. Adverse effects: Macrolides produce very few serious effects, but about 10-20% of patients develop GI symptoms from erythromycin, which may require that a different antibiotic be prescribed. Clarithromycin and azithromycin are usually better tolerated than erythromycin. Erythromycin and clarithromycin are metabolized in the liver, and can inhibit the metabolism of other drugs. Data from surveys in Heilongjiang Province, Inner Mongolia Autonomous Region, and Beijing and Shanghai municipalities are included in this report. These data support finding from previous surveys in other provinces. Heilongjiang Province, and Inner Mongolia Autonomous Region showed 7.2% 95% CLs, 5.9-8.6 ; , and 7.3% 95% CLs, 5.6-9.4 ; MDR-TB among new cases respectively. These proportions are similar to those reported from Liaoning province, also in North Eastern China. Beijing and Shanghai reported lower proportions of resistance; 2.3% 95% CLs, 1.5-3.4 ; , and 3.9% 95% CLs, 2.6-5.6 ; respectively. This is one of the first reports of lower proportions of drug resistance in urban settings. A nationwide survey, based on a random selection of 70 clusters, representing counties or districts, is scheduled to complete in 2008. Surveys in Chongqing, Hunan, and Xinjiang, provinces will be finalized shortly. Despite reaching the global targets for case detection and cure, China has proportions of resistance that are among the highest in the world, only second to rates found in countries of the former Soviet Union. The plan for expansion of MDR-TB treatment under the NTP includes the launch pilot projects in 31 prefectures in six provinces with plans to enroll 5, 000 MDR-TB patients by 2009 and scale up to 50 prefectures in ten additional provinces, treating 10, 000 MDR-TB patients by 2011. Thirty six of the mutations identified in the present study are de scribed for the first time. The newly detected amino acid substitutions were scored for poten tial gross or local conformational changes and influence on molecular stability for every single factor VIII domain with available structures, us ing homology modelling. Synthetic lures were as attractive as well as caged virgin female moths. More moths were captured in the upper portions of trees than in lower portions. Life history studies indicated five instars. In 24-hour trap checking, catches started at 2300 hours and ended at 0500 hours, with a peak at 0100 hours. Area-wide flight phenology studies using pheromone traps in coastal BC, the interior of BC, and the PNW, showed that flights started in early May, peaked in late July to August, and ended in mid-October.
